Thank you for considering to participate in The Ohio State University Animal Influenza Ecology & Epidemiology Research Program's research study. The purpose of the study is to better understand what healthy habit behaviors have been adopted. Please consider the below when deciding whether or not to agree to take part in the study:

  • Participating in the study requires no additional time or effort on your part.
  • By agreeing to participate you are giving the Ohio State study team permission to review your submitted selfies and gather data related to activities you are doing in the photo along with your demographic information. We will be collecting data for the duration of the 2017 swine jackpot show season to observe changes that may occur as you participate in the educational program.
  • Your participation is voluntary. You may refuse to participate or stop participating at any time in this study without penalty or loss of benefits to which you are otherwise entitled.
  • There is no direct benefit to you for participating in this research study. There is a minimal risk of a breach of confidentiality. Efforts will be made to keep the study information confidential. There may be circumstances where this information must be released. For example, personal information regarding your participation in this study may be disclosed if required by state law. Also, your research records may be reviewed by groups at Ohio State responsible for overseeing research studies (e.g., Ohio State University Institutional Review Board and Office of Responsible Research Practices).
